Support Coordination
Support Coordination is funded under ‘Capacity Building’ within your NDIS plan. Here at Fortify Health Group, we will do just that; build your capacity in understanding your NDIS plan and how you can best utilise it.
We understand that ‘building capacity’ can look very different from one person to another depending on so many things; your age, gender, cultural background, language, individual needs and disability. We also recognise the importance of including your support networks, including family and friends, in achieving your goals.
Our Support Coordinators will work creatively and resourcefully with you to give you choice and control over your services, and developing ways to best utilise your support budgets to achieve your goals.
We have Support Coordinators available across Brisbane South, Logan, Gold Coast and the Redlands area, and provide services from our offices at Springwood and Mount Gravatt, as well as outreach in your home or the community.
Your NDIS Support Coordinator will:
- Offer you choices of providers that can meet your needs
- Arrange any assessments required to determine the nature and type of funding required (eg. assessment to support the need or aids and equipment where required or additional funding).
- Work with you to develop a budget for each support type
Level 2 Support Coordination
Our service will work with you to design and find supports that meet your individual needs and help you reach your goals. Your Support Coordinator will assist you to understand your plan, assist in the ongoing management of support, and answer your questions as they arise.
Level 3 Support Coordination
Level 3 support coordination is also known as ‘Specialist Support Coordination’. This will be delivered through a qualified social worker or allied health professional to assist when you are faced with complex situations where you are required to work with multiple government bodies such as Health, Education, and Justice system. We will assist in connecting your NDIS supports with the other service systems you’re working with, negotiating solutions with your multiple stakeholders.
