Clinical Care Coordination

Fortify Health Group will support you with up to 15 free 1:1 clinical support services based on individual needs. The psychological therapies will focus on building psychological and functional capacity, assist you in managing your mental illness and promote an increase in social and economic participation.  Recognizing the complexities and the stigma around mental illness in CALD communities, these supports will be developed in a culturally appropriate manner and interpreters will be offered where required.

Services within MHCCC include:

• Up to 15 individual sessions on a one-on-one basis using evidence-based interventions; delivered by a psychologist;
• Diagnosis, cognitive and functional assessment offered on individual need; and/or
• Using a holistic approach, working with other services to support you in achieving your goals and improving your overall mental health
