NDIS
Your NDIS plan is a document created by the NDIA which contains information about you and your goals. Each of your goals have allocated funds attached that will be in three different areas of your plan – CORE, CAPACITY BUIILDING and CAPITAL.
We will work with you to help understand your goals and individual needs to best cater your NDIS Services to meet your needs.
A Support Coordinator’s role is to understand your needs and goals within your NDIS plan. From there, they will support you in finding the most suitable supports to help you meet your needs.
Whilst providing this support, the Support Coordinator will build your capacity in understanding your NDIS plan to allow you to have more autonomy in the future.
No - A Support Coordinator’s role is to help you understand and implement your NDIS plan and to connect you with the people and services to achieve your goals, while a Plan Manager helps you to manage payment of your NDIS services. Fortify Health Group does not offer plan management services.
Where you NDIS plan is plan-managed, we will work closely with your Plan Manager to organise payments, invoices and manages your budget.
If you have a psychosocial disability, you are eligibility through NDIS to have a Recovery Coach. The Recovery Coach will work with you to understand your individual needs and your personal goals, making sure you have services linked with you that understand you.
We currently are able to offer psychology, counselling and physiotherapy for NDIS participants aged 7 years and above.
Here at Fortify Health Group, our psychologists and counsellors will spend time to understand you and the goals you would like to achieve, and the barriers you are currently facing due to your disability. Our psychologists work within an evidence-based practices and trauma informed framework to best understand the functional impact of your disability and provide you with tools and resources to live the life you choose.
Supports can be provided on an individual basis in one of our offices or in the community. We also offer group supports within our two offices on a weekly basis.
It is likely your first plan review will occur just before your first 12-month plan is due to expire. This is your opportunity to explain what worked well within your plan and what changes you would like to see in in your next plan.
If you have a change in situation and need to trigger for a review prior to the end of your plan, our support coordination/ recovery coach team can work with you to understand your teams, collect supporting evidence, and support you through the process.
Yes, we can provide supports to you if you are agency managed, plan managed and self-managed!

Yes. Where English is not your preferred spoken language, we work with interpreters. Within our team we also speak 24 different languages so an external interpreter may not be needed.
Funded Mental Health Services
Fortify Health Group are funded by Brisbane South Primary Health Care Network to provide supports to adults aged 16- 65 years living with severe and complex mental illness from a CALD background who live within South Brisbane.
This service is not available for those who have NDIS support.
The program has two streams.
- Psychosocial support program - We can support you with up to 10 one-to-one psychological sessions with a psychologist or counsellor. Within this program, if you require an assessment or diagnosis, speak with our team and we will discuss the options available.
- MHCCC - We also have the support of Mental Health Support Facilitators. Our support facilitators can help you by linking you with services to overcome your day-to-day issues. This is inclusive or assisting you to apply for NDIS.
Sadly not. If you are a NDIS participant, you are not eligible for this program.
