Care Services

At Fortify Health Care Services, we prioritise your personal well-being by providing compassionate and individualised personal care. Our dedicated caregivers assist with a range of personal hygiene and grooming activities.

Fortify Health Care Services can provide Supported Independent Living (SIL) services wherever you live. We work with participants to deliver SIL supports and SDA providers to source suitable accommodation if you haven’t already found it.

Our community nursing services ensure that you receive high-quality medical care in the comfort of your home. Fortify Health Care Services employs skilled nurses who provide a wide range of healthcare services.

Our nurses provide a range of nurse-led daily living supports for NDIS and NISSQ participants at home or in the community, promoting health, independence, and well-being for individuals with complex health conditions.

We provide support to ensure that you can participate in life activities that are important to your personal wants and goals, including ensuring you get to appointments on time or to the places and events that are meaningful to your life.

Fortify Health Care Services are able to provide a range of clinical nursing assessments, including continence and CHAPS assessments.

Allied Health & Medical

Our compassionate psychologists are experienced in providing a diverse range of therapeutic strategies including other evidence-based interventions.

Our focus is on delivering personalised care in a respectful and supportive environment, helping you cultivate resilience and foster a stronger sense of well-being.

Our experienced physiotherapists can assist with managing a variety of conditions such as musculoskeletal disorders and neurological conditions.

Whether you’re dealing with depression, anxiety, mood disorders, or other mental health concerns, our team are here to provide expert guidance and support. 

Our experienced and compassionate counsellors can help you to work through challenges in your life. Counselling can assist with a wide range of concerns.

Our social workers provide comprehensive support to improve your personal and social well-being. We assist with navigating the NDIS system.

We offer group therapy supports for people wanting to connect with peers, learn new skills and be supported to maintain their wellbeing in a group setting.

Fortify Health Kids specialises in allied health for children, and has practitioners providing fun, expert care in clinic and in the community.

We offer psychological assessments that diagnose and evaluate the functional impact of mental health conditions or disabilities. 

Care Coordination

We understand that ‘building capacity’ can look very different from one person to another depending on so many things; your age, gender, cultural background, language, individual needs and disability.

We offer psychosocial recovery coaching through your NDIS plan. Our Recovery Coaches help you find services to meet your goals, build your understanding of your plan, and support you with reviews.

Fortify Health Group will support you with up to 15 free 1:1 clinical support services based on individual needs. The psychological therapies will focus on building psychological and functional capacity

CPSP provides non-clinical community-based supports both one-on-one and in small groups to help you achieve your recovery goals. The program may assist you by building capacity and stability in a variety of areas

Training

We offer a range of accredited and non-accredited training courses focused on complex health and disability needs. We have regular sessions on offer, or can tailor support to the needs of your organisation.

What services does Fortify Health Group provide?

We are a registered NDIS and NIISQ provider offering a wide range of health and disability services, including a range of allied health, complex support work and nursing services, assessments,  care coordination, multicultural mental health supports and training courses. We provide services for NDIA-managed, plan-managed, and self-managed NDIS participants. You must have a current NDIS plan with relevant funding to be eligible. Find out more information about accessing the NDIS here: https://www.ndis.gov.au/applying-access-ndis
If you are eligible for Medicare and you have a current referral from your GP, then you may be eligible for Medicare rebates for select allied health and medical services. Ask our team if you need further information.
Some of our services may be claimable through private health insurance, though this depends on your provider. Please check with them to confirm your coverage, especially for counselling and provisional psychology services.
Our fee schedule includes both private and NDIS-eligible services, with Medicare and private health rebates available for eligible participants and services. Please check our fees page or contact us for our latest fee schedule.
No, report writing and letter provision involve an additional cost, which is billed at an hourly rate. Please discuss this with your practitioner if required.

We have free multicultural mental health services at no cost to eligible participants through funding from the Brisbane South Primary Health Network (PHN). Click HERE for eligibility requirements.

We also may provide bulk-billed or subsidised services for some of our allied health or medical services, dependent on need. Find out more about eligibility criteria and how to be added to the waitlist click HERE.

We have offices in Springwood, Mount Gravatt and Ormeau, and provide outreach services in Brisbane South, Logan, Gold Coast and the Redlands. We also will accept referrals from Ipswich and Brisbane North on a case-by-case basis.
We have a skilled multidisciplinary team, spanning a range of disciplines, including psychologists, psychiatrists, social workers, occupational therapists, nurses, physiotherapists, and support staff, all dedicated to providing compassionate care.
We are passionate about providing safe and appropriate services for all of our participants. While we specialise in providing support to people from Culturally and Linguistically Diverse people, we are passionate about providing individualised and safe services that acknowledge and honour each person’s life experiences.
