Support Coordination

Support Coordination is funded under the ‘Capacity Building’ section of your NDIS plan. At Fortify Health Group, we are committed to helping you build your capacity to fully understand and make the most of your NDIS plan.

We know that ‘building capacity’ varies from person to person, depending on factors such as age, gender, cultural background, language, individual needs, and disability. We also value the inclusion of your support network—family, friends, and others—in helping you achieve your goals.

Our Support Coordinators help you manage your services and optimise your budget to meet your goals. We offer support across Brisbane South, Logan, Gold Coast, and the Redlands, from our offices or through in-home and community outreach.

Your NDIS Support Coordinator will:

NDIS Support Coordination

Our service will work with you to design and find supports that meet your individual needs and help you reach your goals. Your Support Coordinator will assist you to understand your plan, assist in the ongoing management of support, and answer your questions as they arise.

Level 3 support coordination is also known as ‘Specialist Support Coordination’. This will be delivered through a qualified social worker or allied health professional to assist when you are faced with complex situations where you are required to work with multiple government bodies such as Health, Education, and Justice system.  We will assist in connecting your NDIS supports with the other service systems you’re working with, negotiating solutions with your multiple stakeholders.

Psychosocial Recovery Coach

A recovery coach works with you to help you find the appropriate services to meet your NDIS goals.

Below are some of the services our Recovery Coach can offer:

  • Spend time with you, and people important to you, to get to know you and understand your needs
  • Discuss different services and providers that maybe suitable to link you with to help you reach your goals
  • Link you with non- NDIS mental health services where applicable
  • Build your capacity in understanding NDIS and your plan
  • Support you with your NDIS plan reviews and planning meetings
     

Our team includes staff with both professional and lived experience in psychosocial disability and mental health. We offer recovery coaching across Brisbane South, Logan, Gold Coast, and the Redlands, with services from our Springwood and Mount Gravatt offices, or through in-home and community outreach.

Hi, I’m Merike.

I am a passionate and dedicated occupational therapist who completed my studies at the University of the Free State, South Africa. My journey in this field began during my Community Service year at a local clinic, where I gained experience in paediatrics, physical rehabilitation, and mental health. I transitioned to private practice, where I further enhanced my skills in physical rehabilitation, mental health, vocational rehabilitation, and hand therapy.

After moving to Australia, I expanded my expertise by working as an NDIS provider, supporting both paediatric and adult clients. This experience allowed me to make a meaningful impact on individuals with diverse needs, empowering them to achieve their goals and improve their quality of life.

I believe every individual has the right to a meaningful and fulfilling life, regardless of their capacity or potential. With a strong sense of responsibility and an eagerness to continually learn, I strive to provide compassionate, dignified care that empowers individuals to overcome challenges and reach their full potential.

Occupational therapy is more than a profession to me; it is a way to help people discover their strengths, regain function, and find purpose. Witnessing clients master new skills, regain independence, and experience an improved quality of life is the most rewarding part of my work.
