Psychosocial Recovery Coach
Fortify Health Group provides psychosocial recovery coaching where it is funded within your NDIS plan.
A recovery coach works with you to help you find the appropriate services to meet your NDIS goals. Below are some of the services our Recovery Coach can offer:
- Spend time with you, and people important to you, to get to know you and understand your needs
- Discuss different services and providers that maybe suitable to link you with to help you reach your goals
- Link you with non- NDIS mental health services where applicable
- Build your capacity in understanding NDIS and your plan
- Support you with your NDIS plan reviews and planning meetings
Our team includes staff with learned knowledge of psychosocial disability and mental health, as well as staff with lived experience.
We have Recovery Coaches available across Brisbane South, Logan, Gold Coast and the Redlands area, and provide services from our offices at Springwood and Mount Gravatt, as well as outreach in your home or the community.
