Fortify Health Group exists for the people, families and teams who find themselves in complex situations with no simple pathway forward. We work where health, mental health, disability and community systems overlap and where gaps, risk or uncertainty can leave people without the coordinated support they need.
Start with the marble
At the centre of the Fortify logo is marble.
It represents the individual not a diagnosis, funding category, referral pathway or service system. Around the marble are two incomplete circles: informal support, such as family, friends and community, and formal supports, such as clinicians, services and professionals.
The message is simple but important: no person should disappear inside the system around them.
Every person is unique.
Every journey is different.
Every model of care should be shaped around the individual, not forced onto them.
For Fortify, “remember your marble” is not a slogan it is a commitment we keep to the person visible in every decision that is made.
More than a support provider
Fortify is not simply another support provider. We act as clinical risk partners for people, families, providers and systems navigating complex care.
We look beyond a roster, referral or single service response and consider the whole environment surrounding the person.
That means asking the questions that matter:
What is driving the risk?
Where is the system breaking down?
What capability is needed around the person?
How do communication, escalation, workforce skill and clinical oversight come together safely in everyday support?
Clinical governance in everyday care
Clinical governance should not sit quietly in a policy folder. It should be visible in the way people are supported every day. At Fortify, clinical assessment connects to individualised planning, workforce training, competency, escalation guidance, multidisciplinary review and access to 24/7 clinical support.
Around 80% of our support workforce are Enrolled Nurses, bringing a strong clinical foundation into our daily support. Our experience includes high-intensity clinical needs, complex medication, acquired brain injury, degenerative conditions, complex psychosocial disability, behavioural complexity, chronic disease and hospital-to-community transitions.
Access, culture and community
We know access is not truly equitable if the pathway only works for people who already understand the system. Fortify brings specialised capability in culturally and linguistically diverse community engagement, cultural navigation and relationship-based access so people can be seen, heard and connected to the right support.
We also deliver the Mental Health Clinical Care Coordination program and the Commonwealth Psychosocial Support Program. These pathways support coordinated clinical care, practical recovery, service navigation and community connection, with referrals available subject to eligibility and pathway assessment.
Different ways to connect and recover
People connect, communicate and recover in different ways. Fortify’s creative therapies provide purposeful opportunities for expression, confidence-building and connection within a broader model of person-centred support. The goal is not to make people fit a rigid service model — it is to shape the support around the person.

Fortify is continuing to grow because the need for connected clinically informed and person-centred support is growing. We are strengthening our clinical capability, partnerships and service pathways so more people can access support that is coordinated, practical and responsive to real life.
