Community Nursing

Fortify Nurse Clinic provides a range of community nursing supports, including Delegation of Care, Disability Related Nursing Supports and Care Planning.

A nurse-led community supports and assessments for people living with complex health conditions.

We offer a variety of courses and programs, created for students of all ages and academic levels. To meet audit requirements, all of our training is delivered by AHPRA Registered Nurses with a training qualification and significant experience.

  • Direct Supports/Procedures

Wound Care – simple and complex dressing changes, reviews and ongoing assessment 

Change or removal of nasogastric (NG) tube, reviews and ongoing assessment 

Insertion, change or removal of indwelling catheters (IDC) reviews and ongoing assessment 

Change of suprapubic catheter (SPC), reviews and ongoing assessment 

High risk medication including injectables via IMI, Subcutaneous or oral routes, infusions and IVT if indicated – order and referral required from GP. 

PEG – reviews and ongoing assessment. 

Complex bowel care including administration of medication and flushes/evacuation. 

  • Delegation of Care

Our Delegation of Care service provides on-going clinical oversight and an on-call function (during business hours) for providers delivering high intensity daily living supports to have access to an RN for care needs. 

The RN will be able to: 

  • Provide personalised training to support workers, families and other stakeholders (Not the same as our certified/formal workshops). Skills could include how to change a catheter bag, seizure monitoring and support skills etc. 
  • Write care plans and train staff/families and other stakeholders in implementation of the care plans. 
  • Provide reports for funding validation and reviews. 
  • Provide nursing supports as listed above. 
  • Be on-call for disability related concerns or needs and give advice or direction – charged at 15-minute intervals. 
  • Review the care provided as per the care plan to ensure it is meeting participant needs, continue to assess and update care plans as required.
