Fortify Health Care Services can provide Supported Independent Living (SIL) services wherever you live. We work with participants to deliver SIL supports and SDA providers to source suitable accommodation if you haven’t already found it.

Comprehensive Care Coordination: Skill nurses who deliver case management to ensure seamless and integrated care across all aspects of your supports.
Household management: Assistance with cleaning, laundry, and organising your living space to maintain a safe and comfortable environment.
Medication Management: Reminders and support to ensure you take your medications as prescribed.
Positive Behaviour Plan Implementation: Implementing strategies as directed by your behaviour support practitioner that assist in managing challenging behaviours and providing feedback to on strategies that are working well for you to your behaviour support practitioner.
24/7 Nurse on call: With a nurse on call around the clock to manage critical health situations and provide immediate assistance when needed.
